Providence Choice is an Oregon based patient-centered medical home plan. You must pre-designate a medical home clinic with Providence prior to receiving services. A primary care physician within that clinic manages your care and makes all referrals to specialists.
If you receive services not coordinated through your designated medical home clinic, those services will be considered out-of-network and higher out-of-pocket costs will be covered at a higher rate.
