Please complete the appropriate e-form below to apply for the staff tuition benefit and submit by the deadlines listed below.


If the form is received after the submission date and you or your family member are enrolled for coursework at another Oregon public university (OSU, EOU, OIT, PSU, SOU, or WOU), the final approval is determined by that university.

Submission of e-form is only accepted for the upcoming term and cannot be accepted for future dated submissions.

Important Deadlines

  • Family transfer forms - two weeks prior to the beginning of each term.
  • Employee tuition forms - two days prior to the beginning to each term.

Beginning fall term 2020, staff tuition forms are completed through an electronic process, which requires a login with your DuckID and password. Paper forms are no longer accepted.
