Return to Work

When returning to work prior to or upon exhausting protected leave, an employee must:

  1. Submit status report.
    Employee submits Employee Status Report, completed by the healthcare provider, to Human Resources (HR) two days prior to returning.
    HR notifies supervisor of intent to return to work status.
     
  2. Return to work.
    Employee and supervisor agree to return-to-work date based on information provided by healthcare provider.
     
  3. Submit final FMLA/OFLA attendance record.
    Employee submits final FMLA/OFLA Attendance Record to HR.
    HR documents the end of the leave period.

Unable to Return to Work

Protected leave is available for a limited period of time. Once the protected leave is exhausted, continuing leave must be discussed with the supervisor, and the employee is encouraged to contact the Employee Labor & Relations representative to discuss accommodation eligibilty. Benefits eligibility may be affected when protected leave ends. Please contact the Benefits Office to discuss options.

The employee must take the following action when protected leave is exhausted and they are unable to return to work:

  • Contact supervisor.
    Contact department supervisor to discuss continuation of unprotected leave.
     
  • Contact Benefits Office.
    Contact the UO benefits office at hrbenefits@uoregon.edu or at 541-346-3085 to discuss continuation of insurance benefits.
     
  • Discuss accommodation eligibility.
    Contact the ADA Coordinator, at workplaceada@uoregon.edu or at 541-346-3159 to discuss eligibility for accommodations under the Americans with Disabilities Act (ADA). This option only applies when on leave for employee's own medical condition.
     
  • Submit FMLA/OFLA attendance record.
    Employee submits final FMLA/OFLA attendance record to HR.
    HR documents the end of the leave period.
