Employee Name and Address Change

Legal Name Change: If you are a past or present University of Oregon employee you must complete the Legal Name Change Form and submit to the University's Payroll Office. This form requires you to attach a copy of your new Social Security Card.

Preferred Name Change: To change your Preferred First Name or nickname, please log in to Duckweb and update it under the Personal Information tab. If DuckWeb is unavailable, you may contact the Payroll Office for assistance. Preferred last name changes are available by request only for professional or safety reasons and can be sent to hrinfo@uoregon.edu. Making this change will update your information in Find People, email, MyTrack, and any other system that uses your preferred name.

Address Change: DuckWeb for university employees allows employees to make address changes online. Login to DuckWeb to update your mailing address.
