Request Leave

Submit your leave request using the web form. You will need the following information to complete the form:

  • UO ID (95#)
  • Supervisor's name and email address
  • Name of the payroll contact for your department

Leave Process

Request Leave

  1. Complete online request form.
    Use the link provided at the top of the page to submit your request to the Benefits Office. Contact hrbenefits@uoregon.edu if you need assistance.

  2. HR reviews form.
    HR reviews the leave request form to determine if the employee meets the minimum qualifications.

  3. HR notifies employee, supervisor and departmental payroll person.
    HR determines employee's eligibility and emails notification to the employee, the employee's supervisor, and the departmental payroll person related to the employee's leave request.

On Leave

While on leave, it is the employee's responsibility to:

  • Keep HR and supervisor informed.
    Inform their supervisor and HR of any changes to the initial leave that are not related to work restrictions (i.e. return to work full schedule and no longer need to take protected leave).

  • Report paid leave use.
    Communicate with their supervisor and departmental payroll administrator if and how they intend to use their accrued paid leave for the first 10 days of leave (i.e. sick, vacation, comp, etc.). The employee is expected to follow the university’s current leave policy to ensure pay can be processed correctly and to prevent a payroll overpayment.

  • Report attendance.
    Submit the FMLA/OFLA COVID-19 Attendance Record to supervisor for review and signature. The supervisor then emails the approved record to< ahref="mailto:hrbenefits@uoregon.edu">hrbenefits@uoregon.edu by the 10th of each month. Failure to submit the required monthly attendance record may result in denial of protected leave.

IMPORTANT NOTE: intermittent leave and a reduced work schedule must be approved by a supervisor.

Return to Work

When returning to work prior to or upon exhausting protected leave, an employee must:

  • Submit final attendance record.
    Employee submits final FMLA/OFLA COVID-19 Attendance Record to HR as soon as possible or by the 10th of the month. Upon receipt, HR documents the end of the leave period.

  • Return to work.
    Employee and supervisor agree to return-to-work date.

Unable to Return to Work

Protected leave is available for a limited period of time. Once the protected leave is exhausted, continuing leave must be discussed with the supervisor, and the employee is encouraged to contact the Employee Labor & Relations representative to discuss eligibility for accommodation. Benefits eligibility may be affected when protected leave ends. Please contact the Benefits Office to discuss options.

The employee must take the following action when protected leave is exhausted and they are unable to return to work:

  • Contact supervisor.
    Contact department supervisor to discuss continuation of unprotected leave.

  • Contact Benefits Office.
    Contact HR Benefits Office at hrbenefits@uoregon.edu to discuss continuation of insurance benefits.

  • Discuss accommodation eligibility.
    Contact ADA Coordinator, at workplaceada@uoregon.edu to discuss eligibility for accommodations under the Americans with Disabilities Act (ADA).

  • Submit final attendance record.
    Employee submits final FMLA/OFLA COVID-19 Attendance Record to hrbenefits@uoregon.edu.
    HR documents the end of the leave period.
