  • Authorization of Transfer of Leave Form (for Hardship Program)
  • Leave Request Form for Domestic Violence, Harassment, Sexual Assault or Stalking Protections
  • Leave Request Form for Non-Protected Leave - (template for use by Departments who wish to officially document non-protected leaves)
